Vermont VPharm Prescription Assistance

Only a few states still run a State Pharmaceutical Assistance Program (SPAP). Medicare Part D, which now caps yearly out-of-pocket drug costs, covers much of what these programs were set up to do. Vermont's VPharm is administered by the Department of Vermont Health Access (DVHA), part of the Agency of Human Services.

What VPharm Covers

VPharm works alongside a member's Medicare Part D plan. It helps pay the Part D plan premium and lowers many drug copays to $1 or $2 per prescription. VPharm is split into three tiers based on income: VPharm 1 (income up to 150% of the federal poverty level) has a $15 monthly premium, VPharm 2 (150.01%-175% FPL) has a $20 monthly premium, and VPharm 3 (175.01%-225% FPL) has a $50 monthly premium.

Who Qualifies

To qualify, you generally need to be a Vermont resident who:

• Is 65 years of age or older, or is under 65 with a qualifying disability
• Is enrolled in a Medicare Part D prescription drug plan
• Has income at or below 225% of the federal poverty level

Vermont also runs Medicare Savings Programs with their own income limits, which may reduce or eliminate the need for VPharm for some members. Check both when you apply.

If You Do Not Have Medicare Part D: Healthy Vermonters

Vermont runs a second program beside VPharm. Healthy Vermonters gives a discount on prescription medicines and has no monthly premium. It is for people who meet the income guidelines and either have no prescription coverage or have used up the maximum benefit on the coverage they have. VPharm is for people who meet the income guidelines and are enrolled in Medicare Part D. The same application covers both.

How to Apply

The form is called the 201P Pharmacy Programs application. There are three ways to send it in.

• Phone: call 1-800-250-8427, Monday to Friday, 8am to 4:30pm, and apply over the phone or ask for a form to be mailed to you.
• Online: download the 201P form, fill it in, sign it electronically and submit it with the state's Document Uploader. Choose "healthcare" as the program.
• Mail: send the signed form and copies of the supporting documents to Green Mountain Care, Application and Document Processing Center, 280 State Drive, Waterbury, VT 05671-1500.

Paying the Premium

VPharm coverage does not start until you pay. If you pay your first bill as soon as it arrives, coverage can begin on the first day of the next month. After that the payment has to reach the state by the due date each month, or you can lose coverage or have a gap in it.

You can pay by check, by credit or debit card, or by automatic deduction from a bank account. Automatic deduction takes time to set up, so expect to pay the first bill or two another way.

Renewing

A renewal form, the 201P Review, comes in the mail. You can return it in the envelope provided, upload it, or renew by phone on 1-800-250-8427. If the state does not have your renewal by the deadline, your benefits end. A form you mail must be signed in pen. A typed signature is accepted only on a form you upload.
